ryeX Medical Form

Each student must have a current ryeX Medical Form on file to participate in events and trips. If your medical history or insurance information changes please complete a new form. The form must be signed by a parent and returned to the Church Office.

Allergy Care Plan

This form must be completed when a student has allergies and staff need to be made aware of a parent's desired care plan. Please return this form to the Church Office.

Asthma Action Plan

This form must be completed when a student has asthma and staff need to be made aware of a parent's desired action plan. Please return this form to the Church Office.

ryeX Youth Ministry Drug & Alcohol Policy

All parents and students agree to the following policy when they register for ryeX trips or events. Drugs and alcohol are strictly forbidden at any ryeX function. Anyone caught with drugs or alcohol will be sent home from the event at their parent’s expense.

Under RPC policy, forbidden substances include, but are not limited to tobacco products, vapes, juules, illegal substances, alcohol and any other controlled substances.

Prescription medications may be turned into adult staff/volunteers OR held by the student as their parent determines most appropriate. All prescription medication must be accompanied by the original prescription container and be issued to the student attending. Please only send enough medication for the number of days/nights your student will be away. Students may NOT share prescription medication with other campers.
